Ealing Adult Community Team for People with Learning Disabilities (CTPLD)

The Ealing Adult Community Team for People with Learning Disabilities (CTPLD) are a team of health and social work professionals , who work in partnership with people with learning disabilities, their carers and other agencies to overcome difficulties and problems.

The CTPLD offers support for adults 18+ who have a diagnosis of learning disability. The team are made up of various professionals such as social workers, speech and language therapists, physiotherapists, Learning Disability nurses, clinical psychologists, a consultant psychiatrist, advanced nurse practitioners and positive behaviour support practitioners. Adults with LD must have an Ealing GP to access the health service and an Ealing residential address to access the social services component of the team.

Referrals can be made by professionals; however self-referrals are also welcomed.  Contact the service directly in the first instance to make a referral.
